Methods: 32 subjects, who suffered from ARDS, have been given hemodynamic monitoring and blood gas analysis, such as cardiac index (CI), systemic vascular resistance index (SVRI),oxygen delivery(DO),oxygen consumption(VO),oxygen uptaking rate(ER)were measured.
目的:探尋中度高原地區(qū)急性呼吸窘迫綜合征(ARDS)的氧缺陷類型;方法:對西寧地區(qū)海拔(2 260m)32例ARDS患者進行血液動力學及血氣分析、測定心排指數(CI)、循環(huán)阻力(SVRI)、氧輸送(DO)、氧消耗(VO)、氧攝取率(ER)以及臨床一般資料進行分析;結果:ARDS血液動力學早期表現為高心排指數、低系統(tǒng)阻力,絕大多數表現高氧耗型氧缺陷,隨著病情進展,成活組CI、SVRI趨于正常,死亡組則表現持續(xù)高動力循環(huán),氧缺陷表現為低氧供型;病程晚期成活組CI、SVRI、DO、VO正常,死亡組仍呈高動力循環(huán);討論:DO下降,但不伴ER的代償性增高,出現低氧供型和病理性氧依賴是ARDS的主要死亡原因。
